Home Health Care Coordinator

Job Summary

Supporting the Home Health Operations team, the full-time Home Health Referral Care Coordinator will coordinate referrals, communicate with providers and injured workers, and ensure timely access to home health and IV therapy services in a fully remote environment.

Key responsibilities

• Manage home health and IV therapy referrals, assisting with intake and documentation as needed
• Promote positive relations with referral sources and ensure accuracy of demographic data and required documentation
• Coordinate with local vendors for services, negotiate rates, and maintain communication with hospitals and insurance carriers for safe discharges

Required qualifications

• Some college education and/or a medical background; LPN preferred
• Experience in Work Comp Insurance, referrals, and authorizations is preferred
• Proficiency in MS Office applications, particularly Word, Excel, and PowerPoint
• Knowledge of medical terminology and injured workers' rights
• Ability to work collaboratively with co-workers and external customers with minimal supervision
