Illinois Licensed Care Coordinator
Job Summary
Providing administrative support for care management activities, the full-time Illinois Licensed Care Coordinator will perform outreach, answer inbound calls, and schedule services for LTSS Waiver members in a fully remote environment.
Key responsibilities
- Conduct outreach to members via phone to assist with care plan next steps and connect them to health plan and community resources
- Serve as front-line support for member and provider inquiries, explaining care plan procedures and protocols
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required qualifications
- High School diploma or GED
- 1 - 2 years of related experience in managed care or healthcare administration
- Knowledge of HCBS waiver services and community resources
- Experience with data entry and member onboarding processes
- Familiarity with Social Determinants of Health (SDOH) needs
