Job Summary

To enhance patient outcomes through proactive healthcare management, the full-time Illinois Licensed Care Navigator will coordinate chronic care plans, track patient data from remote monitoring devices, and educate patients on self-management techniques in a hybrid work environment.

Key responsibilities

• Coordinate chronic care plans for patients in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs
• Track patient data from remote monitoring devices to identify health trends and communicate updates to physicians
• Educate patients about medication, lifestyle changes, and self-management techniques while fostering ongoing engagement

Required qualifications

• Current licensure as a LPN in the state of Illinois or certification as a Medical Assistant through an accredited program
• CPR certification per American Heart Association guidelines required within 30 days of hire
• Familiarity with Chronic Disease Management and Remote Patient Monitoring
• Proficiency in using EHR systems for documentation and care coordination
• Strong organizational abilities to manage multiple patients and tasks effectively
