Job Summary
Providing mental health rehabilitation services, the full-time Illinois Licensed Case Manager will support adults and families in decreasing hospitalization and crisis episodes while increasing community functioning, all in a remote work environment based near Kankakee, IL.
Key responsibilities
- Undertake primary case management responsibilities for designated clients, conducting individual, group, and family counseling sessions
- Maintain communication with staff regarding admissions, processes, and discharge plans, ensuring compliance with regulations for client service records
- Utilize a variety of treatment concepts and evidence-based practices to facilitate recovery goals for clients and their families
Required qualifications
- Bachelor's degree in Psychology, Counseling, Social Work, or a related field, or equivalent experience and training
- Valid driver's license and ability to meet agency insurance requirements
- Self-starter with the ability to work independently and as part of a team
- Proficiency in navigating and documenting in electronic healthcare records
- Ability to complete required documentation within designated time frames
