Illinois Licensed MTM Pharmacist

Job Summary

To support a dynamic Clinical Engagement Services Center, the part-time Illinois Licensed MTM Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and enhance disease state management while working remotely on a contract basis.

Key responsibilities:

• Engage members telephonically to schedule consultations and provide medication therapy management
• Document patient care notes and communicate essential information to primary care providers
• Utilize prescription claims data to generate clinical recommendations and assess medication regimens for effectiveness

Required qualifications:

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of experience working remotely in a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Ability to work a flexible schedule with rotating shifts between 9:00am and 8:00pm CST, Monday through Friday
