Illinois Licensed MTM Pharmacist

Job Summary

To support a dynamic clinical engagement team, the part-time Illinois Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide patient education while working remotely on a contract basis with potential for full-time employment.

Key responsibilities

• Engage members telephonically to schedule consultations and provide medication therapy management services
• Document patient care notes and communicate essential information to primary care providers while adhering to HIPAA regulations
• Utilize prescription claims data to assess medication regimens and generate clinical recommendations

Required qualifications

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of experience working remotely within a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Ability to work a flexible schedule with rotating shifts between 9:00am and 8:00pm CST, Monday through Friday
