Illinois Licensed MTM Pharmacist

Job Summary

To support a clinical engagement team, the part-time contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide patient education while working remotely with flexible hours between 8:00 am and 8:00 pm CST.

Key responsibilities

• Engage members telephonically to schedule pharmacist consultation appointments and provide clinical recommendations
• Document patient care notes and ensure adherence to HIPAA regulations and organizational policies
• Assess medication regimens for effectiveness and educate patients on their medications and disease states

Required qualifications

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the core elements of MTM services
• 1+ years of experience working remotely/independently in a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Ability to work a flexible schedule with rotating shifts as needed
