Job Summary
To support the SNFist Team, the full-time Illinois Licensed SNF Resource Nurse will manage patient telephone interactions, triage calls for new admissions, and serve as a liaison among hospitalists, providers, and nursing homes while working remotely during daytime hours.
Key responsibilities
- Complete necessary telephone communications with patients, families, and healthcare facilities to gather and maintain patient information
- Prepare "pre-charting" information for SNFist providers, including medication reconciliation and summaries of pertinent medical information
- Collaborate closely with SNFist Team nurses, providers, and nursing home staff to ensure timely patient admissions and continuity of care
Required qualifications
- Licensed Registered Professional Nurse (RN) in Illinois
- Basic Life Support (BLS) certification within 30 days of hire from the American Heart Association (AHA)
- Bachelor's Degree in Nursing or related field
- Experience in nursing or healthcare settings
- Professional telephone etiquette and strong communication skills
