Job Summary

Coordinating the Remote Patient Monitoring Program, the full-time hourly Indiana Licensed Telehealth Nurse will oversee patient selection, setup, and training for telehealth monitoring while delivering proactive care interventions remotely.

Key responsibilities

• Manage patient selection and setup for telehealth monitoring systems
• Deliver and evaluate outcomes related to telehealth services
• Provide direct care visits as needed to ensure patient well-being

Required qualifications

• Active Registered Nurse (RN) license with multi-state capability or ability to obtain one
• Minimum of three years' relevant RN experience in an adult acute care setting
• Strong critical thinking skills and ability to form positive patient relationships
• Passion for innovative care models in telehealth
• Preferred: Bachelor of Science in Nursing (BSN)
