Iowa Licensed RN Care Manager
Job Summary
To support maternal health initiatives, the full-time Iowa Licensed RN Care Manager will manage complex care activities for pregnant members in Iowa, focusing on personalized care plans and education, while working remotely Monday through Friday from 8 AM to 5 PM CT.
Key responsibilities:
- Evaluate member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Develop ongoing service plans and collaborate with providers to address unmet needs
- Monitor member progress towards care plan goals and provide education to members and their families on healthcare options
Required qualifications:
- Active Iowa RN license or Compact State Licensure
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2-4 years of related experience in case management and maternal health
- Experience with prenatal or post-partum care is preferred
- Familiarity with OB or NICU care is a bonus
