Journey Patient Specialist
Job Summary
Supporting the Hospice team, the full-time Journey Patient Specialist will manage journey referrals, ensure timely face-to-face visits, and assist with patient communications, while working remotely but based near Troy, MI, Mechanicsburg, PA, or Warrendale, PA.
Key responsibilities
- Develop and process journey referrals, ensuring accurate documentation and workflow
- Manage chart closure processes and provide necessary documentation
- Communicate and collaborate with various healthcare teams for internal coordination
Required qualifications
- High School diploma or GED
- Previous experience as a Journey Counselor or in a Hospice-related role
- Knowledge of medical terminology
- Proficiency in Microsoft Office Suite, including Teams
- Ability to demonstrate excellent customer service and manage concurrent demands
