Job Summary

To support infusion services, the part-time Kansas Licensed Patient Care Coordinator will manage patient intake, coordinate care with healthcare providers, and ensure timely communication and documentation while working remotely.

Key responsibilities:

• Receives and executes the initial patient intake process to ensure timely scheduling of nursing services
• Maintains communication with healthcare personnel and documents care coordination accurately
• Tracks patient visit schedules and assists with staffing for home health agencies

Required qualifications:

• 6+ months of customer service or healthcare-related experience
• Intermediate level of computer proficiency, including Microsoft Office and web applications
• Must be 18 years of age or older
