Kentucky Licensed Care Manager
Job Summary
To support high-quality healthcare outcomes, the full-time Kentucky Licensed Care Manager will develop and facilitate complex care management activities for members with physical needs while working remotely during standard business hours.
Key responsibilities
- Evaluates member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Collaborates with providers to develop ongoing service plans and coordinates care to address member's unmet needs
- Monitors member progress and updates care plans as necessary, ensuring effective communication among all stakeholders
Required qualifications
- Active Kentucky State or Compact RN license
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in medical-surgical, inpatient, outpatient, or care management settings
- Experience with social determinants of health and community resource navigation
- Ability to perform telephonic, digital, and in-person outreach to assess member needs
