Job Summary
To support a leading national healthcare organization, the part-time contract Kentucky Licensed MTM Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and provide patient education while working remotely from Kentucky.
Key responsibilities
- Conduct telephonic consultations to complete comprehensive medication reviews and adherence assessments
- Identify gaps in care and evaluate medication regimens for appropriateness and safety
- Provide patient education and communicate clinical recommendations to members and healthcare providers
Required qualifications
- Active, unrestricted Pharmacist license in Kentucky
- Strong understanding of disease-state management and the core elements of Medication Therapy Management (MTM)
- Minimum 1 year of experience working remotely or independently in a collaborative team environment
- Intermediate proficiency with Microsoft Word, Excel, and Outlook
- Flexibility to work rotating shifts between 9:00 AM and 8:00 PM CST, Monday-Friday
