Kentucky Licensed RN Care Manager
Job Summary
To support transitions of care for members in Kentucky, the full-time Kentucky Licensed RN Care Manager will develop and facilitate personalized care plans, assess member needs, and coordinate appropriate services while working remotely from Monday to Friday, 8 AM to 5 PM CT or ET.
Key responsibilities
- Evaluates member needs and barriers to care, recommending plans for optimal health outcomes
- Develops ongoing care plans and collaborates with providers to address members' unmet needs
- Provides education to members and their families on disease processes and healthcare options
Required qualifications
- Active Kentucky State or Compact RN license
- Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
- 2-4 years of related experience in medical-surgical, inpatient, or outpatient settings
- Experience in care management or home health preferred
- Knowledge of social determinants of health and community resources
