Job Summary
To support transitions of care for members, the full-time Licensed Bilingual Social Worker will provide short-term behavioral health interventions remotely, focusing on connecting members to community-based services and preventing hospital readmissions.
Key responsibilities
- Perform comprehensive psychosocial assessments via phone and video with members and caregivers
- Conduct clinical social work interventions, including brief therapeutic support and navigation of social determinants of health
- Collaborate with interdisciplinary teams to ensure cohesive, patient-centered care for high-needs patients
Required qualifications
- Master's degree in Social Work with an active license in any US state (LCSW, LMSW, or comparable)
- Minimum of 5 years of experience in behavioral health or care management
- Bilingual in English and Spanish
- Competent in performing comprehensive psychosocial assessments over video and phone
- Able to independently manage a fully remote work schedule, M-F, 8:30am-5:00pm CST or PST
