Job Summary

Leading a team of Care Coordinators, the full-time remote Licensed Care Coordinator Manager will oversee patient support throughout their care journey, ensuring high-quality engagement and smooth transitions between levels of care.

Key responsibilities

• Manage and develop a team of Care Coordinators to meet performance and engagement metrics
• Oversee patient activation and support activities, ensuring timely outreach and follow-up
• Coordinate transitions for patients requiring escalation to higher levels of care, partnering with providers and facilities

Required qualifications

• Active clinical licensure with 3-5 years of clinical experience
• 3+ years of experience in healthcare operations, care coordination, or related fields
• Experience managing teams focused on patient outreach and engagement
• Strong understanding of patient care journeys and care transitions
• Proven ability to use performance metrics to improve team effectiveness
