Job Summary
To support clients with behavioral health conditions, the full-time Licensed Care Coordinator, NM will coordinate care through assessments, care planning, and monitoring, working remotely while ensuring quality outcomes and resource management.
Key responsibilities
- Provides care coordination for members requiring intensive interventions and oversight, utilizing clinical, social, and community resources
- Conducts comprehensive health risk assessments and develops care plans, serving as the primary point of contact for service delivery
- Collaborates with interdisciplinary teams to adjust care plans and advocate for members' needs, ensuring effective and cost-efficient care
Required qualifications
- 3-5 years of experience in Social Work, Nursing, or a healthcare-related field, or relevant experience in lieu of a degree
- Experience in utilization management, quality assurance, or community health
- Knowledge of referral coordination to community and private/public resources
- Valid state driver's license
- Preferred certifications include CCM, LCSW, or RN licensure
