Licensed Clinical Pharmacist
Job Summary
To support a growing telehealth initiative, the full-time Licensed Clinical Pharmacist will conduct medication consults, identify adherence barriers, and collaborate with healthcare providers while working remotely.
Key responsibilities
- Conduct medication consults with members to meet quality metrics for medication therapy management
- Identify and address member-specific medication adherence barriers through collaboration with providers
- Support patients in initiating and maintaining new therapies while optimizing medication utilization
Required qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited institution
- Current, unrestricted pharmacist license in the practicing state
- At least 1 year of clinical pharmacy practice experience
- Experience in medication therapy management and comprehensive medication reviews
- Proficient in multitasking, including typing and navigating multiple screens during calls
