Licensed Clinical Pharmacist
Job Summary
To support kidney and cardiac health management, the full-time Licensed Clinical Pharmacist will develop and implement medication initiatives, address pharmacy referrals, and collaborate with clinical teams in a remote setting.
Key responsibilities
- Review and optimize medication regimens based on current guidelines for disease management
- Create educational materials for providers and members regarding medications and treatment updates
- Collaborate with care providers to enhance medication safety and adherence processes
Required qualifications
- Active Pharmacist License
- PharmD degree from an accredited School of Pharmacy
- 5 years of clinical pharmacy experience in hospital or ambulatory care settings
- Experience in renal patient pharmacy management and value-based care preferred
- Understanding of CMS Star measures, HEDIS measures, and MIPS preferred
