Job Summary
To support a growing triage team, the full-time Licensed Clinical Social Worker will provide remote psychosocial assessments, crisis intervention, and care coordination for patients with advanced social needs related to cancer treatment.
Key responsibilities
- Serve as the primary intake clinician for behavioral health and social needs consults, conducting assessments and delivering targeted interventions
- Utilize clinical judgment to determine escalation pathways for crises and coordinate with interdisciplinary teams to address barriers to care
- Maintain high-quality documentation of assessments, safety plans, and referrals while participating in case conferences to support coordinated care
Required qualifications
- Master's degree in Social Work (MSW) from a CSWE-accredited program
- Valid, active, and unrestricted independent clinical social work license in the candidate's current state of practice
- Minimum of 3 years' clinical social work experience in a healthcare setting, with a focus on crisis intervention and behavioral health
- Proven competence in risk assessment and crisis management, with strong telehealth skills
- Excellent clinical judgment and ability to make time-sensitive decisions while working remotely
