Licensed Clinical Social Worker Advocate

Job Summary

To support a growing practice, the Licensed Clinical Social Worker Advocate will provide psychosocial assessments, resource navigation, and behavioral health support to members, working remotely on a 1099 contract basis with flexible scheduling options.

Key responsibilities

• Complete psychosocial and social determinants of health assessments to identify barriers affecting member wellbeing
• Connect members to appropriate community resources and benefits, ensuring follow-up on resource effectiveness
• Provide brief, solution-focused support and coordinate referrals to behavioral health providers when necessary

Required qualifications

• Active, unrestricted LCSW license in at least one U.S. state
• Master of Social Work (MSW) from a CSWE-accredited program
• 2+ years of post-master's experience in medical, community, geriatric, or healthcare social work
• Strong knowledge of public benefits and community resources
• Self-directed with the ability to manage a member load and schedule independently
