Job Summary
Collaborating with an interdisciplinary clinical team, the remote Licensed Clinical Social Worker (LCSW) will provide comprehensive, patient-centered palliative care to individuals with serious illness, focusing on psychosocial support, care coordination, and advance care planning.
Key responsibilities
- Conduct comprehensive assessments to evaluate psychosocial distress, mental health needs, and social determinants of health for patients with serious illness
- Facilitate serious illness conversations and advance care planning with patients and families, ensuring alignment with patient goals
- Deliver short-term clinical counseling and emotional support to patients and caregivers coping with serious illness and end-of-life considerations
Required qualifications
- Master's in Social Work (MSW) from an accredited program
- Active, unrestricted Licensed Clinical Social Worker (LCSW) license in the applicable state(s)
- Clinical experience in palliative care, including advance care planning and serious illness conversations
- Experience working within an interdisciplinary care model with healthcare professionals
- Proficiency in using electronic health records (EHRs) and clinical documentation systems
