Licensed MTM Pharmacist
Job Summary
To support a leading national healthcare organization, licensed MTM Pharmacists will engage in telephonic consultations, conduct comprehensive medication reviews, and improve patient outcomes in fully remote contract positions.
Key Responsibilities
- Conduct telephonic consultations to complete comprehensive medication reviews and adherence assessments
- Identify gaps in care and evaluate medication regimens for safety and efficacy
- Provide patient education and communicate clinical recommendations to members and healthcare providers
Required Qualifications
- Active, unrestricted Pharmacist license in the state of residence
- Strong understanding of disease-state management and MTM core elements
- Minimum 1 year of remote or independent work experience in a collaborative environment
- Ability to navigate multiple clinical platforms and applications simultaneously
- Flexibility to work rotating shifts between 9:00 AM and 8:00 PM CST, Monday-Friday
