Licensed MTM Pharmacist
Job Summary
To support a leading national healthcare organization, licensed MTM Pharmacists will engage in telephonic consultations, conduct comprehensive medication reviews, and improve patient outcomes in a fully remote contract position.
Key Responsibilities
- Conduct telephonic consultations to complete comprehensive medication reviews and adherence assessments
- Identify gaps in care and evaluate medication regimens for potential issues, including drug interactions and inappropriate dosing
- Provide patient education on medications and communicate clinical recommendations to members and their healthcare providers
Required Qualifications
- Active, unrestricted Pharmacist license in your state of residence
- Strong understanding of disease-state management and the five core elements of Medication Therapy Management (MTM)
- Minimum 1 year of experience working remotely or independently within a collaborative team environment
- Ability to navigate multiple clinical platforms, databases, and applications simultaneously
- Flexibility to work rotating shifts between 9:00 AM and 8:00 PM CST, Monday-Friday, with occasional extended hours or weekends
