Job Summary
Providing vital support to patients with complex needs, the full-time Licensed Palliative Care Social Worker will conduct telephonic and video visits, develop individualized care plans, and collaborate with interdisciplinary teams in a remote setting.
Key responsibilities
- Conduct psychosocial assessments and create personalized care plans tailored to members' needs
- Facilitate goals of care conversations and advanced care planning discussions, including hospice transitions
- Provide emotional and caregiver support while navigating complex family and support issues
Required qualifications
- Master's degree in social work and active license
- Minimum of 2 years of experience in palliative or hospice care
- Proficiency in leading goals of care conversations and assisting with advanced care planning
- Certified hospice and palliative social work credential, or willingness to obtain within one year
- Strong interpersonal skills and comfort working with patients over video and phone
