Licensed Pharmacist - Patient Consultation

Job Summary

To support a growing specialty pharmacy, the part-time Licensed Pharmacist - Patient Consultation will conduct telephonic clinical consultations, perform medication reconciliations, and provide education on medication use and disease management while working remotely.

Key responsibilities

• Conducts telephonic clinical consultations to ensure safe and effective use of specialty medications
• Performs patient assessments and medication reconciliations to identify adherence barriers and medication-related concerns
• Documents patient consultations and clinical recommendations in accordance with company policies and regulatory requirements

Required qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a bachelor's degree in Pharmacy or PharmD
• Current, unrestricted licensure as a Registered Pharmacist in the state of residence
• Experience counseling patients and caregivers regarding medication therapy and disease state management
• Experience documenting patient assessments and clinical interventions according to pharmacy practice standards
• Experience evaluating medication regimens for potential drug interactions and adherence barriers
