Job Summary
Supporting a multidisciplinary care team, the full-time Licensed Social Work Case Manager will coordinate services for patients with complex chronic conditions, focusing on care plans that address socioeconomic and psychosocial needs while working remotely.
Key responsibilities
- Establish trust and build strong relationships with patients through telephonic and virtual engagement
- Collaborate with an interdisciplinary team to ensure cohesive, patient-centered care for high-needs patients
- Facilitate advanced care planning discussions and assist in navigating complex family and support issues
Required qualifications
- Master's degree in social work and an active state license
- Minimum of 3 years of experience in medical social work, case management, or care coordination
- Bilingual in Spanish preferred
- Health insurance experience preferred, particularly with Medicare Advantage
- Experience working with the senior population is a plus
