Job Summary

To support the Population Health and Health Services area, the full-time Louisiana Licensed Care Coordination Nurse will assess, plan, implement, monitor, and evaluate healthcare services for members, ensuring quality and cost-effective outcomes while working remotely.

Key responsibilities

• Assess member needs and develop comprehensive disease management plans
• Coordinate and implement interventions to achieve established healthcare goals
• Conduct medical necessity reviews and manage utilization of benefits effectively

Required qualifications

• High School Diploma or equivalent required
• 4 years of direct patient care experience required
• Current, unrestricted LPN license in Louisiana required
• Knowledge of medical terminology and ICD 10 codes required
• Experience in managed care or claims processing preferred
