Job Summary
To support a dynamic clinical engagement team, the part-time contract Louisiana Licensed MTM Pharmacist will perform telephonic consultations, manage comprehensive medication reviews, and ensure adherence to clinical guidelines while working remotely.
Key responsibilities:
- Engage members telephonically to schedule consultations for clinical programs
- Utilize prescription claims data to provide clinical recommendations and assess medication regimens
- Document patient care notes and participate in departmental meetings to enhance clinical programs
Required qualifications:
- Current and unrestricted Pharmacist license in Louisiana
- Strong knowledge of disease state management guidelines and MTM services
- 1+ years of remote work experience in a cross-functional team environment
- Proficiency in Microsoft Word, Outlook, and Excel
- Bachelor's Degree or Pharm D
