Job Summary

To support a growing healthcare initiative, the full-time Louisiana Licensed Population Health RN will organize and provide care coordination and case management services for members at risk of health deterioration, working remotely while ensuring compliance with regulations and quality standards.

Key responsibilities

• Assess and develop comprehensive case management plans for members, advocating for their healthcare needs
• Coordinate with members, providers, and community resources to implement and evaluate care plans effectively
• Participate in quality improvement projects and meet individual performance standards for program effectiveness

Required qualifications

• Active and unencumbered RN license to practice in Louisiana
• Diploma, Associate's, or Bachelor's degree in nursing
• Three years of recent direct patient care/clinical experience
• Experience in managed care is preferred
• Certified Case Management Certification is preferred, required within three years of hire
