LPN Care Coordinator

Job Summary

To support injured workers with low to moderate complexity claims, the full-time Care Coordinator Specialist will coordinate medical care, facilitate communication among stakeholders, and assist in return-to-work efforts under RN supervision.

Key Responsibilities

• Coordinate routine medical care needs and facilitate communication between injured workers, providers, and employers
• Monitor adherence to treatment plans and maintain accurate documentation of case progress
• Manage administrative functions including scheduling appointments and attending team meetings

Required Qualifications

• High School Diploma required
• Graduate of an accredited practical/vocational nursing program
• Current compact LPN/LVN license in good standing
• 2+ years of clinical experience required; workers' compensation experience preferred
• Proficiency in Microsoft Office Suite and case management systems
