LPN Clinical Outreach Specialist

Job Summary

To support high-risk patients during their transition from hospital to home, the full-time remote LPN Clinical Outreach Specialist will conduct patient outreach, perform medication reconciliations, and collaborate with primary care practices to close care gaps and prevent readmissions.

Key responsibilities

• Conduct targeted patient outreach and follow-up to ensure timely care transitions and close care gaps
• Perform medication reconciliation and provide patient education while documenting interventions across multiple EHRs
• Collaborate with practices to support care gap initiatives and enhance patient engagement strategies

Required qualifications

• Certificate or Diploma in Practical Nursing or Associate Degree in Nursing (ADN)
• Current and unrestricted LPN license
• Minimum of 2 years clinical experience in case management or discharge planning
• Experience with telephonic patient outreach and clinical assessments
• Demonstrated ability to prioritize and manage high-acuity cases
