Maine Licensed Social Worker

Job Summary

To support Maine residents with disabilities facing complex healthcare and service coordination challenges, a hybrid Licensed Social Worker position will provide case coordination, community collaboration, and advocacy to ensure access to appropriate services and safe living environments.

Key Responsibilities

• Provide consultation and support to clients and care teams addressing complex healthcare, financial, and housing issues
• Facilitate multidisciplinary team meetings to develop safe discharge plans and identify service barriers
• Maintain accurate documentation and track case activity in compliance with departmental protocols

Required Qualifications

• Current Licensed Social Worker (LSW) credential issued by the Maine State Board of Social Worker Licensure
• Six years of education and/or professional experience in social services casework or related fields
• Experience in case management or care coordination
• Knowledge of healthcare systems and community-based service delivery models
• Understanding of client rights and ethical practice standards
