Maryland Licensed Case Manager
Job Summary
To support a growing healthcare team, the full-time Maryland Licensed Case Manager will coordinate care and health education for patients with chronic conditions, working remotely during standard business hours.
Key responsibilities
- Identifies and addresses changes in patient conditions and implements corrective actions
- Reviews financial reports related to patient populations and maintains budgeted caseloads
- Collaborates with staff to ensure effective care plans and resource utilization
Required qualifications
- Current registered nurse licensure in Maryland or LCSW-C
- Bachelor's degree in nursing or related experience, or MSW in social work preferred
- Minimum five years of clinical practice experience
- Three years of clinical practice in a specialty area preferred
- Certification in case management (CCM) preferred
