Job Summary
Supporting high-risk patients in a newly established case management program, the full-time Maryland Licensed Case Manager RN will coordinate care, provide chronic disease education, and engage patients in improving self-management while primarily based in Westminster with some remote work eligibility after 90 days.
Key responsibilities
- Coordinate care across multiple providers and settings for patients with complex medical and social needs
- Provide chronic disease education and perform medication reconciliation for patients
- Implement programs aimed at improving health outcomes and reducing unnecessary healthcare utilization
Required qualifications
- Bachelor's Degree in Nursing (BSN) is required
- Minimum of 2 years of experience in clinical nursing is required
- Current Maryland RN License or multi-state/compact license is required
- Experience in managed care is preferred
- Experience in a community-based acute care setting is preferred
