Maryland Licensed RN Case Manager
Job Summary
To support a growing team, the full-time Maryland Licensed RN Case Manager will coordinate care and health education for members with chronic conditions, working remotely during standard business hours.
Key responsibilities
- Identifies and acts upon impending changes in member conditions to ensure appropriate care
- Analyzes issues impacting care plans and initiates corrective actions as necessary
- Maintains a budgeted caseload while ensuring member goals are achieved within specified timeframes
Required qualifications
- Current registered nurse licensure in the state of Maryland or LCSW-C
- Bachelor's degree in nursing or related field, or MSW in social work preferred
- Minimum five (5) years of clinical practice experience
- Minimum three (3) years of clinical practice in a specific specialty area
- Certification in case management (CCM) preferred
