To enhance patient outcomes in palliative care, the full-time Maryland RN Telehealth Coordinator will manage the telehealth program, collaborate with care teams, and oversee operational aspects while working remotely.
Key responsibilities
- Facilitates patient assessments and triage through the telehealth platform, ensuring quality palliative care delivery
- Champions program improvements and serves as a liaison for implementing new initiatives to enhance patient care quality

Required qualifications
- Bachelor's degree in Nursing required; Master's degree in Nursing preferred
- 10+ years of clinical experience in a setting caring for older adults
- Active RN license in the District of Columbia or State of Maryland required upon hire
