Job Summary
To support a multidisciplinary clinical team, the full-time Massachusetts Licensed Ambulatory LPN will deliver high-quality, patient-centered care through remote modalities, focusing on clinical triage, medication education, and ongoing patient follow-up.
Key responsibilities:
- Conduct telephonic and video-based patient triage to assess symptoms and clinical concerns
- Provide comprehensive patient education on medications and lifestyle interventions
- Document clinical encounters accurately within the EMR, ensuring compliance with protocols
Required qualifications:
- Current and active LPN license in Massachusetts
- Minimum of 2 years of clinical experience, preferably in ambulatory or telehealth settings
- Proficiency with telehealth technologies and remote patient communication
- Strong communication and patient education skills
