Massachusetts Licensed Care Navigator

Job Summary

To support patient engagement in a remote setting, the Massachusetts Licensed Care Navigator will triage vital signs, manage patient wellness calls, and collaborate with care teams to promote health behavior changes while working full-time within the EST time zone.

Key responsibilities

• Manage physiological markers such as blood pressure, weight, and blood glucose with clinical appropriateness
• Conduct monthly wellness calls and lead collaborative discussions to define health goals with patients
• Provide preventive health education and coaching while meeting patient engagement program goals

Required qualifications

• Unrestricted RN/LPN/LVN license in a compact state and Massachusetts
• 1-3 years of clinical experience in patient management or disease management
• Experience performing virtual visits and telephonic care management
• Ability to work independently with minimal direction
• Spanish speaking is a plus
