Massachusetts Licensed MTM Pharmacist
Job Summary
To support a dynamic Clinical Engagement Services Center, the contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and document patient care while working remotely with flexible hours between 9:00am and 8:00pm CST.
Key responsibilities
- Engage members telephonically to schedule consultations and provide adherence-related support
- Utilize prescription claims data to generate clinical recommendations and assess medication regimens
- Document patient care notes and participate in departmental meetings to enhance clinical programs
Required qualifications
- Current and unrestricted Pharmacist license in the state of residence
- Strong knowledge of disease state management guidelines and MTM service elements
- 1+ years of experience working remotely in a cross-functional team environment
- Intermediate proficiency with Microsoft Word, Outlook, and Excel
- Bachelor's Degree or Pharm D
