Job Summary

To support a digital-first primary care practice, the part-time Massachusetts Licensed Nurse Practitioner will provide high-quality, comprehensive care through asynchronous text-based chat and synchronous video, managing complex and chronic illnesses for a diverse patient population.

Key responsibilities

• Provide best-in-class care for patients managing complex and chronic medical conditions
• Utilize technology and data to optimize patient outcomes while ensuring personalized care
• Collaborate with team members to efficiently solve patient problems and foster an inclusive culture

Required qualifications

• 5+ years of advanced clinical experience in complex care, chronic care management, or primary care
• Board certified as a Nurse Practitioner with an active NP license in Massachusetts
• Experience or willingness to learn about digital-first healthcare practices
• Proficiency in using technology and applications for virtual patient care
• Proficiency with written and spoken medical Spanish is a plus
