Maternity Care Coordinator

Job Summary

Serving as the single point of contact for the Mom and Baby Beginnings program, the full-time remote Maternity Care Coordinator will engage and enroll members, conduct health assessments, and coordinate care services while providing quality customer service and maintaining accurate documentation.

Key responsibilities:

• Telephonically engage and enroll members in the Mom and Baby Beginnings program
• Conduct initial screenings and needs assessments, considering cultural and linguistic needs
• Coordinate care and collaborate with internal teams and healthcare providers to develop Person-Centered plans

Required qualifications:

• Diploma or Associates in a healthcare-related field or equivalent work experience required; Bachelor's degree preferred
• A minimum of three years of experience in healthcare, including case management or care coordination
• Intermediate proficiency with Microsoft Office applications
• Knowledge of healthcare laws and case management practices
• Strong understanding of cultural and demographic diversity
