Medication Navigation Specialist

Job Summary

To support patient transitions in medication management, the full-time remote Medication Navigation Specialist will guide members through infusion site transitions, manage prior authorizations, and coordinate with healthcare providers while developing navigation workflows and quality standards.

Key responsibilities

• Manage end-to-end transitions from hospital outpatient infusion to ambulatory or home settings, including site vetting and provider coordination
• Conduct outreach to members for medication optimization and guide them through the process
• Serve as the primary liaison between members and care teams, coordinating prescription changes and tracking adherence

Required qualifications

• 2-5 years of experience in an outpatient infusion setting in a coordination or administrative role
• Hands-on experience with prior authorizations and insurance coverage
• Strong communication skills for explaining complex medical and pharmacy benefits
• Comfort working with AI-assisted workflows and technology platforms
• Startup mentality with the ability to build processes from scratch
