Job Summary
To provide high-quality, cost-effective healthcare outcomes, the full-time Michigan Licensed Care Manager (RN) will develop, assess, and facilitate complex care management activities for members with primarily physical needs while working remotely.
Key responsibilities
- Evaluate member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Develop ongoing care plans and collaborate with providers to address unmet needs and ensure timely access to services
- Monitor member status and progress, revising care plans as necessary to meet individual goals
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current unrestricted Michigan RN license
- Experience in developing and implementing care plans
- Familiarity with social determinants of health and community resources
