Job Summary

To support a growing remote healthcare team, the part-time Michigan Licensed Care Navigator will manage patient engagement through wellness calls, triage physiological markers, and collaborate with care teams to improve health outcomes while working in the EST time zone.

Key responsibilities

• Manage physiological markers like blood pressure, weight, and blood glucose with clinical appropriateness
• Perform monthly wellness calls and lead collaborative discussions to define patient health goals
• Provide preventive health education and coaching while ensuring patient engagement in remote patient monitoring programs

Required qualifications

• Unrestricted RN/LPN/LVN license in a compact state and Michigan
• 1-3 years of clinical experience in patient management or disease management
• Experience with remote patient monitoring and telephonic care management
• Ability to work independently with minimal direction
• Spanish speaking is a plus
