Job Summary
Supporting members through assessment and care coordination, the full-time temporary Michigan RN Nurse Case Manager will conduct comprehensive member assessments, develop individualized care plans, and engage in telephonic outreach while working remotely.
Key responsibilities
- Conduct comprehensive member assessments and develop individualized care plans
- Coordinate care with interdisciplinary teams, providers, and community resources
- Document care management activities accurately and ensure compliance with regulatory requirements
Required qualifications
- Active and unrestricted Michigan Registered Nurse (RN) license
- Associate's Degree in Nursing (ASN) or related field
- Minimum 3+ years of nursing experience
- Minimum 2+ years of case management or home healthcare coordination experience
- Experience providing care management for Medicare and/or Medicaid members
