Job Summary

To support home health operations in Winona, Minnesota, the full-time Minnesota Licensed Patient Care Manager will oversee patient care delivery, manage staff, and ensure compliance with healthcare regulations while working onsite.

Key responsibilities

• Manage patient care services and coordinate with healthcare teams to ensure quality care
• Supervise and support staff, providing guidance and training as needed
• Ensure compliance with Medicare Conditions of Participation and relevant regulatory requirements

Required qualifications

• Current Minnesota RN licensure
• Minimum of two years of management or supervisory experience in a home care setting preferred
• Knowledge of professional standards and practices in home health care
• Understanding of federal, state, and local regulatory requirements
