Minnesota Licensed Telehealth Physician

Job Summary

To support a growing telehealth initiative, the full-time Minnesota Licensed Telehealth Physician will build strong relationships with patients and healthcare advocates, oversee patient care plans, and contribute to a new patient care program in a remote setting.

Key responsibilities

• Learn and utilize Solace systems and tools while providing clinical expertise in patient interactions
• Identify and prioritize patients' needs to maintain a streamlined care continuum
• Oversee patient care plans in collaboration with healthcare advocates addressing social determinants of health

Required qualifications

• Board certification in family medicine, internal medicine, or other relevant specialty
• Active DEA license and HAP
• Strong clinical and organizational skills with the ability to manage multiple tasks under pressure
• Experience working with diverse patient populations, including the elderly and those facing social determinants of health
• Technical proficiency and adaptability in learning new systems and software
