Minnesota Licensed Triage Nurse

Job Summary

Working on an on-call/PRN basis, the part-time Minnesota Licensed Triage Nurse will provide telephone triage support for Assisted Living and Hospice teams, primarily during evening shifts, while ensuring accurate documentation and adherence to clinical standards.

Key responsibilities:

• Independently provide professional nursing services over the telephone, documenting symptoms, assessments, and advice given
• Follow up on high-risk situations and analyze data to improve service outcomes
• Maintain clinical standards and ethics as per the Nurse Practice Act and participate in quality improvement activities

Required qualifications:

• Must be licensed as a Registered Nurse in Minnesota
• At least 1 year of care management experience in home care or hospice
• Strong clinical and relational skills
• Ability to analyze situations and implement effective solutions
• Proficient with Microsoft Office Suite
